Key Responsibilities:

• Following the client’s written plan of care and carrying out duties as assigned by your supervisor.

• Observing the client’s functioning and reporting any changes to the Care Manager.

• Conducting essential shopping and running errands.

• Maintaining accurate records of daily activities and service hours.

• Providing or assisting with personal care, including:

  • Routine housekeeping tasks, such as laundry.

  • Meal preparation.

• Escorting clients to appointments.

• Lifting up to 30 lbs, standing for extended periods, and climbing stairs as needed.

• Traveling to clients’ homes and assisting with public transportation.

Qualifications:

• Experience in home care or related fields is preferred.

• Ability to lift 30 lbs and perform physical tasks.

• Strong organizational and communication skills.

• Reliable, compassionate, and dedicated to improving clients' quality of life.

• Must have reliable transportation.
